fine particle cast

Introduction

Introduction Under certain conditions, the protein and cells or fragments filtered out by the kidneys are solidified in the renal tubules (far-curved) and collecting tubes, and can form a cylindrical protein aggregate and be discharged with urine, which is called a tube type. A fine particle tube type is one which contains more fine and sparse particles.

The fine particle tube needs to be distinguished from the various tube types below.

The transparent tube type is a colorless uniform translucent cylinder. 27% of normal people can have a transparent cast in the urine, which is of little clinical significance. This type of tube is seen in the urine during strenuous exercise, kidney stimulation, and ether anesthesia.

White blood cell tubular type of white blood cells or pus balls arranged in a roller shape, suggesting the presence of acute and chronic pyelonephritis, acute pyelonephritis often accompanied by bacterial casts. Leukocyte casts can also be found in non-renal substantial infectious diseases such as streptococcal infectious nephritis, membrane proliferative nephritis, and active lupus nephritis.

The red blood cell tube type contains a plurality of red blood cells. When red blood cells are lysed into reddish brown particles, they are called blood casts. Both red blood cell casts and blood casts suggest intrarenal hemorrhage, which can be seen in acute and chronic glomerulonephritis, acute tubular necrosis, renal infarction, and renal transplant rejection.

Renal tubular epithelial cells tubular tube type inner tubular epithelial cells are regularly arranged, suggesting that they are from the same part of the renal tubule, while irregular arrangement is suggested to come from different parts of the renal tubule, which means that the renal tubule is damaged.

The granular tube type transparent tube type contains particles, and the amount of the tube type is more than 1/3 of the tube type, which is called a particle tube type, and is generally classified into a fine particle tube type and a coarse particle tube type. The former tube type contains more fine and sparse particles, which are found in the late stage of chronic glomerulonephritis or acute glomerulonephritis; the latter is coarse and dense, and is found in renal tubular damage caused by chronic glomerulonephritis or drugs and heavy metal poisoning.

The fat tube type contains a large amount of fat droplets, which is common in nephrotic syndrome.

The waxy tube type is large in appearance and even in color. It is found in the late stage of chronic nephritis, renal amyloidosis, and chronic renal failure.

Bacterial tube type refers to the tube-type transparent matrix containing a large number of bacteria, more common in acute pyelonephritis.

The fungal tube type contains spores, hyphae or pseudohyphae, and red blood cells, white blood cells or renal tubular epithelial cells are often seen. It is found that the fungal cast type can diagnose primary and disseminated mold infections early.
