Anal itching at night

Introduction

Introduction Ascariasis is an intestinal parasitic disease that causes the anus and perineal itching to be special. Ascariasis is a common parasitic disease in which children are transmitted to each other. The main symptoms are itching around the anus, even worse at night, the child is irritated, sleepless, or scratching itching with hand rubs, which can cause inflammation of the perianal area or other skin diseases and insomnia, enuresis and other diseases.

Cause

Cause

Causes:

The disease is caused by a child swallowing a mites. The food, clothes, and toys that children eat may be contaminated with eggs, and when they enter the body with food, they will develop tsutsugamushi. Mature female aphids lay eggs near the anus of the sick child at night, causing itching. If the child reaches for the anus, his fingers and nails will be stained with some early eggs. If the child sucks his finger or takes something without washing his hands, he will be infected again. The mites will also be passed on to the family by contaminated food, cups, sheets and towels.

Adults parasitize the cecum, appendix, colon, rectum and lower ileum of the human body. When severely infected, they can also be parasitic in the upper part of the small intestine and even in the stomach and esophagus. The worm body adheres to the intestinal mucosa by the action of the head flap and the lip flap, or is in a free state in the intestinal lumen. Adults feed on intestinal contents, tissues or blood. After mating of females and males, the female uterus is filled with eggs and migrates to the lower part of the intestinal lumen. When a person sleeps, some females move outside the anus. Due to changes in temperature and humidity and oxygen stimulation, a large number of ovulations begin to occur, and the eggs are adhered to the perianal skin. Females after ovulation often die due to dryness, but a few females can move back to the intestine by anal peristalsis. If you enter the vagina, uterus, fallopian tubes, urethra or abdominal cavity, pelvic cavity, etc., it can lead to ectopic parasitism.

Such as the anus around the perineum or the perineum often itchy, children with irritability at night, should pay attention to the possibility of tsutsugamushi, if you can find the worm, eggs can be diagnosed.

First, the egg test method: due to the special spawning habits of aphids, the positive rate of eggs detected in feces is less than 5%. There is an examination of the anorectal eggs.

1. Wiping method: Put the cotton swab in sterile saline first, wring it out when it is used, wipe around the anus, and mix it on the glass slide with 50% glycerin solution. You can also use the flat head of the toothpick to infiltrate with 50% glycerin or 1% sodium hydroxide solution, then use it to wipe the wrinkles around the anus, scrape the scraping material with a cover slip, and apply it on the slide. 1 spectroscopy of 1% glycerol or 1% potassium hydroxide.

2. Floating method: use a cotton swab in physiological saline, squeeze it out, wipe the anus around, then put the cotton test tube into a test tube with saturated saline, shake it thoroughly to wash the eggs into the salt water, and then float the eggs. Perform a microscopic examination.

3. Scotch tape wiping method: Before the defecation, use the transparent adhesive tape to wipe the skin around the anus. Check it under the microscope at low magnification for 3 times, the positive rate can reach 79.4%. This method is simple and suitable for census applications.

Second, the worm examination method: the child examines the anus 1 to 3 hours after falling asleep. If the worm crawls out, it can be stored in a vial filled with alcohol with a scorpion. Because the mites do not necessarily climb out of the ovulation every night, they need to observe for 3 to 5 consecutive days.

Diagnosis

Differential diagnosis

Identification:

Most of the disease has only mild abdominal pain, such as severe pain, abdominal muscle tension, should go to the hospital immediately to eliminate appendicitis, peritonitis or other acute abdomen caused by aphids. Aphids cause itching of the perineum, similar to symptoms caused by perineal fungal infections, allergies and eczema; aphid urinary tract inflammation is similar to general urinary tract infections, and should be noted. Combined with age, disease characteristics and local signs, there is no difficulty in clinical diagnosis. Anal examination of aphids or eggs is a direct evidence of differential diagnosis.
