Esophageal Diverticulectomy

The esophageal diverticulum is a blind pocket that covers the epithelium with the esophageal lumen. Congenital esophageal diverticulum is clinically rare. May be a variation of esophageal repetition. The true diverticulum should have a mucosa, submucosa, and esophageal muscle layer, starting with a small blind pocket, which later increases due to the mechanism of a similar bulging diverticulum. Congenital diverticulum can occur at any level of the esophagus. Common sites are the pharyngeal tube and the esophagus. Any part of the esophagus or any type of diverticulum should be removed as long as the pouch develops to a clinically symptomatic condition. Treatment of diseases: congenital esophageal diverticulum esophageal diverticulum Indication The pharyngeal esophagectomy is suitable for diverticulum enlargement, saliva and food reflux, cough and other symptoms. Preoperative preparation 1. Esophageal barium meal or esophagoscopy examination of lesions and size. 2. Oral esophageal anti-inflammatory drugs. 3. Enter the fluid diet 1 to 2 days before surgery. Place the stomach tube before surgery. Surgical procedure (a) cervical esophageal diverticulum resection 1. Position: The supine position, the head turns to the right. 2. Incision: oblique incision of the anterior border of the left sternocleidomastoid, 8 cm long. 3. Cut the platysma, retract the sternocleidomastoid muscles outward, and pull the scapulae muscles inward. 4. Reveal the huge diverticulum, clamp the bottom with a pliers, and pull a little to free the diverticulum to the base of the neck. 5. Cut the muscle layer around the root of the neck to reach the submucosa. 6. Retain the 2 to 3 mm wide edge and remove the diverticulum. The mucosa is closed by a suture of a thin thread. 7. The suture is sutured by a thin wire. (2) Upper esophageal diverticulum resection 1. Position: The left side is lying. 2. Incision: 7th intercostal incision in the posterior aspect of the left chest. 3. After dissociating the lower esophagus, pull the diverticulum. 4. The diverticulum resection method is the same as above.
