Forearm Flap Excision

The forearm temporal flap is generally used. The flap contains iliac, vein, cephalic vein and forearm lateral cutaneous nerve and superficial branch of the radial nerve. The latter remains in the forearm. The radial artery projection line is from the midpoint of the intercondylar sacral line of the iliac crest to the iliac artery of the radial stump of the wrist. The flap is designed as the central axis, and the upper end can reach the lower 1/3 level of the upper arm. It can reach the transverse ligament of the wrist, and the width of both sides can reach 3/4 of the circumference of the forearm. The forearm flap has a constant blood vessel, a thin thickness, and a large donor area. It is especially suitable for the repair of facial and hand wounds. However, due to the exposed parts of the forearm, there are still ugly traces after suturing or skin grafting. This flap is forbidden when the ulnar artery is not well supplied. Surgical procedure The lower edge of the flap is opened, and the sacral, venous, and cephalic veins are exposed. A longitudinal incision is added to remove the blood vessel to the length required for the vascular pedicle. The ligation and cutting can be performed first for later operation. The sides of the flap were cut to the superficial musculature, and the flap was sharply peeled with a sharp-edged knife. The ulnar side reached the superficial flexor tendon of the iliac crest, and the temporal side reached the superficial temporalis, lifting the vascular pedicle and skin. At the lower end of the valve, after peeling from the lower to the upper part of the iliac artery to the masking part, the blood vessel is peeled off from the deep part of the muscle abdominal space until the upper edge of the flap, the upper side edge is cut, and the blood vessel is cut and ligated. It is also possible to continue to peel the blood vessel up to the desired length as needed. Be careful not to separate the blood vessels from the flaps along the way.
