Anterior Orion Plate Fixation

Anterior Orion plate fixation is used for the treatment of spinal fracture and dislocation. Treatment of diseases: cervical vertebrae wedge compression fracture cervical spine fracture and dislocation Indication Anterior Orion plate fixation is suitable for: 1. Single-segment dislocation injury without bone defect or vertebral compression fracture requires interbody fusion. 2. For the anterior column of the vertebral body due to severe wedge-shaped compression fracture or burst fracture, accompanied by neurological dysfunction, anterior decompression and bone grafting are required for vertebral body resection. Double-segment fixation is optional. Surgical procedure Reveal More transverse incisions are used, and the vertebral bodies and intervertebral discs are exposed according to the anterior approach, and decompression or bone grafting is performed as needed. Remove traction to check the stability of the bone graft. 2. Select steel plate The vertebral body proliferative bone spur was repaired to make the steel plate placed more closely. The screw points at both ends of the steel plate should be as close as possible to the vertebral body bone graft bed, which is advantageous for the angle of the two ends of the screw to the head and tail of the nose 15 °. It also prevents the steel plate from exceeding the adjacent intervertebral space. 3. Adjust the steel plate curvature as needed The steel plate itself has a curvature suitable for the lordosis of the cervical vertebra, but if the curvature is not suitable, the curvature can be adjusted by a special steel plate bender. It should be noted that the operation should be gentle and the force should be uniform to prevent the sharp angle of the steel plate. It is worth emphasizing that the change of the curvature of the steel plate will inevitably affect the angle of the screws at both ends of the steel plate, and attention should be paid during operation. 4. Place the steel plate The plate is placed in the anterior midline of the vertebral body with a wrench, and the position of the uncinate joint of the vertebral body can be used as a reference point. 5. Drilling Insert the drill sleeve first, the correct position is 6° to the center line, and the angle to the two ends is 15°. Once the position is correct, the handle of the drill sleeve can be lightly pressed, and the drill sleeve can be safely locked into the steel plate. Care must be taken to ensure that the drill sleeve is along the edge. The longitudinal axis of the steel plate is oriented. Insert the appropriate drill bit into the drill sleeve to a depth of 13 mm and over 13 mm to operate with a drill bit of adjustable depth. 6. Tapping Remove the drill sleeve and tap the drill hole at the same angle with the matching tap and the same depth as the drill hole. 7. Screw in the screw You can use the depth detector to measure the screw hole depth, select the appropriate screw length, and use the matching 6-point screw driver to screw the appropriate length screw (Fig. 3.26.5.15-3A), but not completely tighten, and then follow the similar operation method. Screw the screw into the diagonal screw hole at the other end of the plate. Then add the remaining screws in turn. The wrench can be used throughout the operation, but it can usually be removed after inserting 1 or 2 screws. If it needs to be used again, it is convenient to insert temporarily. Finally tighten the screws one by one so that the nut is lower than the surface of the steel plate. The position of the steel plate and the depth of the screw can be checked with a C-arm X-ray machine. 8. Screw in the bone graft screw For multi-planar interbody fusion, or longer support bone graft, a 4.35mm gold screw can be screwed into the diagonal groove at the center of the steel plate. Optional 11mm, 13mm, 15mm three kinds of specifications, should be screwed vertically, the drill sleeve is fixed on the diagonal groove of the steel plate, first drilled, tapped with 4.35 tap, and then screw in the screw. 9. Install the locking screw Use a locking screw clamp to insert the locking screw into the steel plate and screw it in properly, then replace the special screw driver to tighten it. Do not use a clamp to tighten the screws, as this will damage the tool. Because the starter has an automatic setting device, the starter will automatically slide out after the screw is tightened. 10. Suture incision Wash the incision with saline, indwell the drainage, and suture the incision in layers.
