High ligation, extraction or varicose veinectomy of the small saphenous vein

High saphenous vein ligation, extraction or variceal resection for lower extremity varicose vein surgery. Lower extremity varicose veins are a common disease with the highest incidence of peripheral vascular disease in China. Lower extremity varicose veins refer to the superficial venous system of the lower extremitiesthe great saphenous vein, the small saphenous vein, and their branches are in an elongated, sacral, and dilated state. Very few symptoms in the early stage, long-term venous congestion can be secondary to phlebitis, skin dystrophic changes, pigmentation and ulceration. Treatment of diseases: varicose veins of the lower extremities Indication High saphenous vein ligation, extraction or variceal resection is suitable for primary small saphenous vein. Large saphenous vein varices, such as small saphenous varicose veins, need to deal with small saphenous veins after treatment of great saphenous vein. Contraindications 1. Acute thrombophlebitis of deep calf and superficial veins. 2. Deep vein thrombosis is formed. Preoperative preparation 1. Carefully examine the function of the small saphenous vein valve and the valve function of the traffic branch between the small saphenous vein and the deep vein. If there is a traffic valvular insufficiency, the position of the body surface should be calibrated. 2, prepare the armpit and calf skin, use gentian violet to draw the varicose veins. Surgical procedure 1. Incision Make a transverse incision at 2 cm above the axillary skin fold, about 5 cm long. 2, high ligation Cut the skin, subcutaneous tissue, straight to the fascia, cut the fascia, and find a small saphenous vein between the two points of the gastrocnemius at the junction of the middle and outer iliac crest. All branches should be ligated and cut. The small saphenous vein was ligated at the confluence of the iliac vein, the proximal end was double-ligated, and the distal end was temporarily clamped. 3, small saphenous vein extraction An incision was made at the ankle varices to find a small saphenous vein, which was ligated and cut. A stripper with an elliptical head is inserted proximally and exits from the distal venous end of the armpit. Ligation of the vein end on the stripping rod and slowly pulling it down. The incision was sutured and pressure bandaged. 4, varicose small saphenous vein resection If there is no residual varicose veins after extraction or extraction, it can be removed. Cut the skin, make a sneak separation under the skin, ligature and cut off the traffic branch, peel off the small saphenous vein of the varicose veins, and suture the incision.
