Treating diseases: alopecia areata Indication 1. Hair thinning or lack of small pieces caused by various reasons, such as male pattern baldness, scarring alopecia, and eyebrow removal. 2. The donor area (mostly the pillow area) originated well. Contraindications 1. Scar physique. 2. Inflammatory skin diseases, such as chronic radiation dermatitis, suppurative skin disease, recurrent herpes simplex, acne with obvious inflammation, and xeroderma pigmentosum. 3. Bleeding tendency. 4. Mental illness. 5. Severe visceral disorders. 6. Vitiligo activity period. Preoperative preparation 1. Preoperative conversation, clarify the purpose, methods and possible complications, as well as precautions, and improve various examinations (see skin grinding for details). 2. Cut the hair in the donor area, wash and dry, and spray antibiotic solution to disinfect. 3. Hair or eyebrow design. Surgical procedure 1. Main equipment. Surgical incision bag, ring drill, No. 7 injection needle, plucking tweezers, hair-planting needles and push-pins. 2. Local routine disinfection, infiltration anesthesia. 3. Single planting. Suitable for bald brows and small pieces of baldness. First, remove the individual hairs with complete hair follicles and hair bulbs in the direction of hair growth with plucking tweezers in the donor area, and then use a 7-gauge needle to punch holes or directly implant the designed hairs with a needle. Each plant is separated by about 1mm. After the planting is completed, the field is covered with Vaseline gauze, and then covered with sterile gauze and tape. 4. Columnar implants. First take the fur piece in the donor area, then divide it into a hair column or a ring drill directly to take the fur column, each column containing 3 or 4 pieces of hair. Drill holes in the planting area with a ring drill, take out the original fur-free column, and then implant the fur column to suppress hemostasis and dressing. 5. Free skin grafting. The skin pieces of the same size or slightly larger area are respectively taken in the hair supply area and the hairless area, and the fur piece is implanted into the hairless area, continuously or intermittently sutured, packaged and fixed, pressurized and bandaged, and the elastic cap is externally fixed. 6. Proximal repair after skin expansion. Use a soft tissue dilator to expand the normal scalp adjacent to the hair in the bald area to obtain enough skin to repair the bald area. See the Cosmetic Surgery section for details. 7. Postoperative treatment (1) High occipital position 1 week after surgery to reduce facial edema and subcutaneous blood stasis. (2) Avoid heavy physical activity 2 to 3 days after surgery. (3) Use antibiotics within 1 week after surgery. (4) Avoid direct sun exposure within 2 months of transplantation.
