Thyrohyoid cystectomy

In the early stage of the embryo, the thyroid primordial root descends from the tongue with a pedicled tubular structure along the midline, that is, the thyroid tongue tube, and the residual of the tube becomes the cause of the thyroglossal cyst. The thyroglossal cyst can be located in the inferior or medial line of the neck, on the hyoid bone, under the hyoid bone, or in the thyroid. The cyst is round, with clear boundaries, thin walls and mucus. In the secondary infection, there is purulent secretion in the cystic cavity, and the fistula is formed after the ulceration. Treatment of diseases: thyroglossal cyst Indication The thyroglossal cyst should be surgically removed, and it is generally appropriate for surgery over 1 year old. If the abscess is formed, the drainage is first cut, and the operation is performed after the infection is controlled to form the fistula. Surgical procedure 1. Incision and cyst exposure A transverse incision along the skin line is made on the surface of the neck cyst, which is generally 3 to 4 cm long. The subcutaneous tissue and the platysma can be cut open, and the sublingual muscles such as the sternohyoid muscles can be separated from both sides to reveal the cyst. 2. Separation and resection of cysts Separated from the anterior, inferior and bilateral sides of the cyst, the pedicle is often connected to the hyoid bone at the top. The upper and lower edges of the hyoid bone are attached to the genioglossus and sternum. In the middle part of the hyoid bone, the muscle is sharply separated from the tongue, and the central part of the bone is 0.5 to 1 cm. The free hyoid bone is still connected with the cyst. Continue to trace the ductal tissue between the hyoid bone and the blind hole of the tongue and remove it with the cyst. If the pharynx is cut below the base of the tongue, it needs to be sutured with a fine absorbable suture. 3. Suture incision The end of the hyoid bone does not have to be sutured. After the wound cavity is actually stopped, the suture is layer by layer. Generally, the drainage can be omitted, and if necessary, a small rubber piece can be taken out from one end of the slit.
