Complication

Physical complications Complications

It can be complicated by bacterial infections, and the body mites caused by microsporum canis and gypsum-like microspores are more common in the face, neck and upper limbs, often accompanied by head lice, possibly because the patient scratches the scalp or combs the hair to spread the above parts. Causes infection.

Symptom

Because the bacteria invade the epidermis, they usually only parasitize the stratum corneum, so they only cause mild inflammation in the local area. They start with red papules or small blisters, and then form scales, which then gradually expand into a ring-shaped lesion with clear boundaries. It is often seen that papules, blisters, the surface is generally free of liquid, the edges are active, constantly expanding, and the center tends to subside, so there is a round or money, and some ring-shaped papules can appear in the ring skin lesions. Blisters, scales, and concentric ring damage, with varying degrees of itching.

Skin lesions vary with pathogens. The disease course of the body mites caused by Trichophyton rubrum is often more prolonged and more common. It is more common in the waist and abdomen, buttocks and trunk. It may have itching, and the pigmentation is often left behind. The body mites caused by Trichophyton rubrum invade the exposed parts of the cheeks and extremities, showing a ring shape or irregular shape, local inflammation is more obvious, itching is obvious, and bacterial infection can occur concurrently. The body sputum caused by flocculent sputum is more common in the lower back. Irregular shape, local inflammation is light, scales are thin and less, and itching is not obvious. The body mites caused by Microsporum canis and gypsum-like microspores are more common in the face, neck and upper limbs, often accompanied by head lice, possibly due to The patient scratches the scalp or combs the hair to spread the above-mentioned parts and causes infection. The body damage caused by the purple hairy fungus is often small (more than 5 cents), and the number is often multiple, often reddish papules. Then, it expands into a round shape around the periphery, with more edge pimples and small blisters.

There are many subtypes in the body.

Examine

Physical examination

Direct microscopic examination can be diagnosed after finding fungal hyphae or spores.

Histopathology: commonly used PAS staining, hyphae and spores can be found in the stratum corneum of the epidermis. Mycelium grows in parallel with the stratum corneum. When there is granulomatous damage, hyphae can be found in the hair follicle and upper part of the dermis, and the epidermis can be parakeratized. Excessive keratinization, thickening of the spinous layer, edema of the dermal papilla, infiltration of inflammatory cells around the blood vessels, and sometimes blister formation under the stratum corneum or epidermis.

Diagnosis

Physical diagnosis

It is mainly determined according to the clinical manifestations. If necessary, the dandruff can be scraped from the edge of the lesion for direct microscopic examination. After the fungal hyphae or spores are found, the diagnosis can be confirmed.

Differential diagnosis

1. Rose pityriasis: easy to be confused with body phlegm, but no disease above the neck, the skin lesions are oval, with marginal scales, inflammation is more pronounced, the long axis of the rash is consistent with the skin pattern, often the mother spot appears first, the fungal test is negative .

2. Seborrheic dermatitis: occurs in the fatty area, the damage is subacute inflammation, the surface is greasy with small scales, itching is severe, and the fungal test is negative.

3. Psoriasis vulgaris: The rash is sometimes ring-shaped, but the base is light red infiltration, covered with multiple layers of silvery white scales, a little bit of bleeding after stripping, occurs in the extremities of the extremities and articular surface, negative fungal examination.

In addition, it needs to be differentiated from chronic eczema, neurodermatitis, and simple pityriasis.
