Oral cholecystography

Oral cholecystography is a commonly used method for the diagnosis of gallbladder disease. For this examination, a drug called gallbladder contrast agent is administered orally. The commonly used contrast agents are iopanoic acid and iodine acid. After the drug is absorbed into the body, it is mainly excreted by the liver through the bile, so it contains a contrast agent. Bile enters the gallbladder from the bile duct, and after enrichment of the gallbladder, the gallbladder is filled with bile containing a high concentration of contrast agent. Basic Information Specialist classification: Digestive examination classification: X-ray Applicable gender: whether men and women apply fasting: fasting Tips: On the day before the angiography, take a high-fat diet for lunch and a small oil diet for dinner. Normal value The normal gallbladder develops better at 12-15h after taking the drug, and the density is uniform. The gallbladder is often densely shaped with eggplant or pear shape, 7-10 cm long and 3-4 cm wide, located below the lower right margin of the right upper abdomen. Clinical significance Abnormal results Oral cholecystography is a method for determining the function of gallbladder enrichment. This method can be used to diagnose gallbladder disease, especially the theoretical basis for the diagnosis of cholecystitis. 1. Can observe the shape of gallbladder and gallbladder lesions, as well as the function of gallbladder. 2. Suspected cholelithiasis and no positive findings in the flat area of the gallbladder. 3. The bile duct development rate of the oral method is 50% to 70%, which can be simultaneously developed with the gallbladder, or can be developed 20 to 40 minutes after the fat meal. Need to check the population suspected of gallbladder-related conditions. Precautions 1. Severe liver and kidney dysfunction, it is not appropriate to repeat the examination or double dose examination. 2. The second photography time of the gallbladder concentrated image can be determined according to the development situation. 3. To increase the contrast agent concentration in the gallbladder, a double dose method can be used. 4. When the contrast effect of oral contrast agent is not good, intravenous injection can be used instead. 5. May trigger the onset of cholecystitis. Inspection process Photography position (1) The patient is lying prone on the camera platform, and the right abdomen is raised so that the coronal plane between the human body and the table top is about 20°. (2) The upper edge of the film includes the right diaphragm, the lower edge includes the ankle, the inner edge includes the midline of the spine, and the outer side includes the right abdominal wall. (3) The center line is vertically injected into the cassette through the gallbladder triangle. (4) Use a filter. (5) The shooting distance is 100 cm. (6) breath holding exposure. 2. Photography procedure (1) After oral contrast agent, photography was started after 14 hours. (2) In the photo, the gallbladder shows satisfaction and then takes a fat meal. (3) The systolic function image was taken from 45 minutes to 60 minutes after the fat meal. (4) The photographic technician carefully fills in the relevant items and technical parameters of the inspection application form and signs them. Not suitable for the crowd Acute gastroenteritis, pyloric obstruction. Adverse reactions and risks May cause an infection.
